Temporary Work Permit Application Forms

Please read the Working Temporarily in Canada section of our website and the instruction guide provided below carefully before you complete the application forms. The fee for processing your application is not refundable, so make sure you are eligible before you apply. Please note that submitting a completed application and necessary supporting documents does not guarantee that you will receive a permit.

Step 2: Review the Temporary Work Permit Instruction Guide and Application Forms on the CIC website

Step 3: You MUST submit the following forms with your application. If you do not submit all forms, your application will be returned as incomplete:

  • Application for Work Permit Made Outside Canada [IMM 1295] | PDF * (347 KB)
  • Family Information [IMM 5645] | PDF * (59 KB)

Step 4: You must also submit the following forms, if applicable to your case (see the Instruction Guide above to see if these apply to you):

  • Schedule 1 - Application for a Temporary Resident Visa Made Outside of Canada [IMM 5257 - Schedule 1] | PDF * (61 KB)
  • Use of a Representative [IMM 5476] | PDF * (55 KB)
  • Statutory Declaration of Common-law Union [IMM 5409] | PDF * (97 KB)
  • Live-In Caregiver Questionnaire: This form must be completed by the Principal Applicant if applying as a Live-In Caregiver.
