If your passport has been lost or stolen, you need to report it immediately to the passport issuing office nearest you. If the loss/theft occurred in the jurisdiction of Hong Kong or Macao on a weekday, please telephone (852) 2847-7348 or fax to (852) 2847-7561. If the loss/theft occurred after office hours or on the weekend in the jurisdiction of Hong Kong or Macao, please call (852) 2847-7575 and an Operations Officer at the Emergency Services Division in Ottawa will assist you. This officer is available 24 hours a day, 7 days a week.
For each lost/stolen passport, you must report the matter to the nearest police station and obtain a detailed police report. You will be required to submit the original police report in support of your application. After reporting the lost/stolen passport to our office or to the Operations Officers and the police, you may begin preparing an application form for issuance of a new passport if you need to travel. You may also request a copy of the form to be mailed to you. Please provide your full mailing address and a daytime contact telephone in case we need to reach you. You are advised to submit the forms in person.
