Lost / Stolen Passport

If a passport is lost or stolen within Malaysia, the bearer of the passport is requested to report to the local police and obtain a copy of a police report. The holder should contact the High Commission of Canada in Kuala Lumpur the next working day for full information on replacement procedures.

In case of urgency, the holder should report to the High Commission the next working day with the police report, three photos of the correct size (many studios are open on Sundays) and appropriate fees.

The individual must then fill out a new application form as well as other related forms. The High Commission must obtain authority from the Headquarters before a replacement document can be issued.

Statutory Declaration in Lieu of Guarantor

If a guarantor is not available, the applicant must complete a Statutory Declaration in Lieu of Guarantor - Form PPT 132. This declaration must be completed and signed before a Commissioner for Oaths or the Consular Officer of the High Commission.
