Applying to Passport Canada

Canadian residents of the USA can choose to use any one of three different downloadable application forms.Application forms may also be obtained by calling Passport Canada at (800) 567-6868, by collecting them in-person from the Consular Services Section of the Consulate General of Canada (New York), or by requesting them from the Consulate by email

Which of the three forms will be best depends upon individual circumstances.

Simplified Form (PPTC 054)

The form can only be used by those who are eligible. Applicants using this form will be asked for less information than other forms, do not need to obtain a guarantor, and do not need to submit proof of citizenship with the application (though any Canadian passport issued within the past 6 years must be submitted).

Standard Form (PPTC 153/155) and "Old Form" (PPTC 140/142)

Applicants ineligible for the Simplified Form, can use either the Standard application form or the "Old" application form. The principal difference between the "standard" and "old" form is with the choice of guarantor. For the "standard" forms 153/155 the guarantor must be a Canadian citizen who is in possession of a valid 5 year Canadian passport, who has known the applicant for two years or more, and who lives in same country as the applicant. For the "old" forms 140/142 the guarantor can be of any nationality but must have known the applicant for two years or more and be employed in a profession deemed eligible to serve as a guarantor (e.g. chiropractor, dentist, judge, lawyer, magistrate, medical doctor, notary public, pharmacist, police officer, veterinarian, signing officer of a bank which offers the full range of retail banking services)

If no eligible guarantor can be found, a Statutory Declaration in Lieu of Guarantor must be completed. This form can be obtained from the Consular Services Section of the Consulate General of Canada (New York), in person or be requested by email.

Required Documents

There is a requirement to submit supporting documents along with applications. Please read and follow the instructions carefully.

Photographs must be obtained and adhere to detailed photo specifications. Many photographs submitted to Passport Canada are rejected as they do not meet requirements. Please read the specifications closely.

For your convenience, the Consulate General has compiled a list of photo studios in the tri-state area and Bermuda that have the specifications for and routinely produce Canadian passport photos. It may be wise to bring a copy of the photo specifications to the studio to ensure that photos obtained meet all requirements.
