Study Permit Application Forms
Please read the Studying in Canada section of our website and the instruction guide provided below carefully before you complete the application forms. The fee for processing your application is not refundable, so make sure you are eligible before you apply. Please note that submitting a completed application and necessary supporting documents does not guarantee that you will receive a Study Permit.

You must submit the following completed forms:
- Study Permit Checklist Please complete and place this checklist on top of your application.

- Live-In Caregiver Checklist
- Live-In Caregiver Questionnaire This form must be completed by the Principal Applicant if applying as a Live-In Caregiver.
