There are two methods of obtaining the Revenue Canada tax forms. You can telephone Revenue Canada on 613-952-3741. They will accept a collect call from Poland. Their fax number is: 001-613-941-2505.

Income tax in Poland
The Government of Canada and Poland have concluded a Convention for the avoidance of double taxation with respect to taxes on income for persons who are residents of one or both of the Contracting States. Please visit Department of Finance Canada for more details on the Convention.
