Passports - Frequently asked questions (Section 7)
The Embassy of Canada in Rome has prepared a list of frequently asked questions to assist you with Section 7 of your application. You should write to us only if you do not find an answer below.
Address history
- Remember to check the box "Same as current address" if this is your case.
- You must cover the previous two years - use an additional page if necessary.
- You MUST indicate dates of start and end for each address.
- If, at some point, you haven't had a fixed address, you may use the postal address where your mail was being sent to.
Occupation in the last two years
Even if you are a homemaker or retired, you must indicate since when this is your occupation and cover the previous two years.
Gaps in employment or schooling should also be listed, with start and end dates, as "unemployed", "sabbatical", etc. as the case may be.
Mother's maiden name
This means your mother's surname at birth; please, do not provide her given name(s).
