It must reflect the exact charges for services requested and be completed in Canadian dollars.
Failure to complete the form in full or authorize the correct fees may result in delays to your application.
You may use one authorization form for more than one service or application; make sure to mark the total amount and the name(s) of all applicants.
Payment for citizenship services (such as the request for a Certificate of Canadian Citizenship) must be done directly online. A copy of the receipt obtained must be sent together with the application.
Payment for passport services must be done directly online. A copy of the receipt obtained must be sent together with the application.
