Lost and Stolen Passports

Please immediately call the Embassy if you are in Italy or San Marino, the Consulate in Malta or that in Albania. After office hours, contact the Emergency Watch and Response Centre (EWRC) at +1 613 996-8885. Alternatively, calling the Embassy in Rome and following the prompts for Canadians in distress (option 1, then 2) will redirect your call to the EWRC after hours. You should also file a police report at the nearest police station.

Most applicants will be required to appear in person at our offices. Please discuss, with the consular agent in charge of your file, your travel schedule and whether you have an eligible guarantor in the country.

The issuance of a replacement travel document is entirely at Passport Canada’s discretion. Under normal circumstances, we are usually able to issue a Temporary Passport or an Emergency Travel Document on the second or third business day after we receive a complete application. You may need to reschedule your flights accordingly, but we advise only doing so after a representative of the Government of Canada has assessed your situation.

Honorary Consuls in Italy, Malta and Albania are not employees of the Government of Canada. As a consequence, they may have professional obligations and commitments causing them to be absent from their office. We suggest you call before going to a Consulate, to confirm that service can be offered and to book an appointment.

You will minimally be asked to provide:

  • A completed application form; links and instructions can be found on our dedicated webpage
  • Two photos taken by a photographer in the last 6 months (those taken at photo-booths are not accepted). Please read carefully the instructions concerning the format of the photos. There are photographers near all of our offices
  • Fees – at the Embassy in Rome it is possible to also pay by Visa or MasterCard
  • Proof of imminent travel, in the form of a ticket or electronic confirmation
  • The Statutory Declaration concerning a lost, stolen, damaged, destroyed or inaccessible Canadian passport (PPTC203)
  • The Statutory Declaration in lieu of Guarantor, if you do not have an eligible guarantor in the country. Note that this declaration (which is not available online) must be signed before a Consular Officer, and that the references indicated on this form must differ from those on the Passport application form. You are being asked to provide a total of four references, whom we must be able to contact on the phone prior to issuing your travel document

If possible, you should also provide:

  • A police report
  • Valid photo ID, or photocopies thereof, if available
  • The original of your proof of citizenship, i.e. a Canadian birth certificate or Certificate of Canadian Citizenship. If the original of this document is not with you but available to someone, please ask them to bring it, along with their own personal ID, to a Passport Canada office or a diplomatic representation of the Government of Canada. The document will be validated there and transmitted to us via secure electronic channels to form part of your application.
