Instructions for an Adult Abroad General Passport Application

Ensuring that your application is completed as thoroughly as possible will help us process it without delay. Incomplete applications will not be processed and will be returned.

Step 1: Download, complete, print, date and sign:

  • Adult Abroad General Passport Application* PPTC040 (PDF, 587 KB).
  • Ensure you sign the completed application on all pages and that your signature is within the green box on page 1.
  • References:
    • Must have known you for at least two years;
    • Cannot be related to you (this includes in laws and partners);
    • May reside in any country and be of any nationality.
  • Ensure your guarantor is acceptable.
  • Prior to your guarantor witnessing and dating your application, you should have completed all of the forms, as well as signed and dated each page.
  • If you do not have a guarantor, include a completed Statutory Declaration in Lieu of Guarantor form, which can be obtained by sending an email to:

Step 2: Gather supporting documents:

  • Include a proof of Canadian citizenship:
    • Your original Canadian birth certificate or;
    • Your original Canadian citizenship certificate (it must show your date of birth).
  • Include at least one supporting document to confirm your identity:
    • A piece of valid ID other than your previous passport or proof of citizenship.
  • Include two identical photographs taken within six months:
    Important: it is highly recommended that you closely examine your photos prior to submitting your application to ensure that the passport photo specifications have been met.
  • Include your last Canadian passport (regardless of expiry date).

Step 3: Include payment:

  • You may omit the postage fee if your application includes a self-addressed registered return envelope that includes postage.

Step 4: Submit your application, choose one of the following:

  • By post:
    High Commission of Canada
    Passport Section
    Canada House
    Trafalgar Square
    London SW1Y 5BJ
  • Via the drop-off box:

  • In person, at the counter:
